Most likely you need some sort of government issued ID, like a state ID card or a learner's permit, driver's license, or pport. If you don't have any of those, then a birth certificate and school ID (with photo) are enough for you (they need to establish that you ARE you, and no some one claiming to be you). Your mother needs government issued ID, if you do not have the same last name then a birth certificate will be required OR paperwork showing her legal name change (like marriage papers if she remarried), basically she has to prove she is who she says she is AND that she is your legal parent or guardian.
